What is this form for?

This form allows you to report any breach, concerning situation, or incident that you have observed or experienced in connection with Café du Cycliste's activities—whether within our company, our supply chain, or our retail locations.

We are committed to handling every report seriously, confidentially, and without retaliation.

How to complete the form

The form contains five sections. Here is how to complete them:

Stakeholder identification

Indicate your relationship with Café du Cycliste: employee, supplier, customer, partner, local resident, etc. You do not have to provide your name if you wish to remain anonymous.

Nature of the report

Select the category that best matches your situation: human rights, working conditions, environment, discrimination, business practices, or other.

Description of the facts

Describe the situation as precisely as possible: what you observed, when and where it happened, and who was involved. The more detailed your description, the more effectively we can act.

Attachments (optional)

If you have documents, photos, or any other material that can support your report, you may attach them directly to the form.

Contact details (optional)

If you would like to be kept informed about the follow-up to your report, you may provide an email address. This information will remain strictly confidential.

Processing

What happens after submission?

Each report follows a handling process defined in our internal procedure:

Within 5 days

Acknowledgment of receipt and registration of the report, if you provided contact details.

Within 15 days

Analysis of the situation and, if necessary, the launch of an internal or external investigation.

Within 30 days

Response and implementation of corrective measures. You will be informed if you provided contact details.
